How do I schedule an appointment at one of the Cancer Centers?
Whether you are a patient making your own appointment OR a physician making a patient referral, you can start the registration process simply by phoning one of our clinics nearest you.

Go to: Find a Clinic.

• Please have the following information handy:
  • Diagnosis
  • Location of medical records, x-rays, and scans to be obtained
  • Names of referring and primary care physicians
  • Demographic information (name, phone numbers, date of birth, address, insurance information, medical record number, etc.)
  • Insurance information will be verified. Many managed care plans have specific requirements that must be met prior to consultation and treatment (authorizations, referrals, etc.)
• After this registration process, an appointment will be scheduled promptly with a Radiation Oncologist

Suggestion:
Before your first visit, take a moment to fill out New Patient documents and bring them with you.
